What information do you collect or store?
From Members:
Account logins are based on eligibility information provided by each employer, and
may contain a subset of personally-identifying information. Members may contact
866-9-CAM-PPO (866-922-6776) for details on the generation of their user ID. The
member’s first and last name, as provided by the employer, are stored as part of
their account information in a centralized database. Members have the option of
additionally associating an e-mail address with their account login.
From Providers:
Provider usernames are generated from a unique provider ID and may contain identifying
information. Providers may contact 866-9-CAM-PPO (866-922-6776) for details on the
generation of their user ID. Provider name, website URL, and National Provider Identifier
are associated with the account. Additionally, office location details - including
street address, city, state, postal code, phone number, comment, practice tax ID,
and practice name – are collected for one or more of the provider’s locations and
linked to their account. Members or other providers may also complete a provider
nomination form for non-network providers, submitting provider name, phone number,
website URL, street, city, state, postal code, and specialization details. Providers
may also optionally associate an e-mail address with their website account on-line
through Provider Services or by providing it durring the application process.
How will you use this information?
For Members:
Information stored in the member’s login ID or otherwise associated with the account
will only be utilized for the purpose of site authentication and customer service.
Member e-mail addresses collected through the website will be used for account management
purposes – for example, e-mailing a new password if the previous password is forgotten.
From time to time, we may also e-mail updates regarding the website or the CAM PPO
in general. Members that do not wish to receive these e-mails may opt out when providing
or updating their e-mail address.
For Providers:
Provider nomination information may be used to contact the nominated provider by
phone to initiate the credentialing process and is subject to verification. Provider
name; website URL; and location address, phone number, comment and practice name
become part of the on-line provider directory, viewable by all members and providers.
NPI and practice tax ID are maintained for internal purposes and will not be displayed
in the directory. Provider e-mail addresses collected through the website or application
process will be used for account management purposes – for example, e-mailing a
new password if the previous password is forgotten. From time to time, we may also
e-mail updates regarding the website or the CAM PPO in general. Providers that do
not wish to receive these e-mails may opt out when providing or updating their e-mail
address on-line.
Will you share this information with other parties?
Membership and provider e-mails are maintained for the exclusive use of the CAM
PPO. We do not sell e-mail lists to third-party organizations. When nominating a
provider, members and providers have the option of requesting that their name be
disclosed to the nominee. We will hold the nominator’s name in confidence unless
they explicitly request otherwise. Active providers will have basic demographic
information published in our on-line directory (see above). If a user requests to
see the results of a directory search on a map, this basic information will be posted
to a server operated by Yahoo! for the purpose of mapping those locations. We will
not disclose NPI or tax ID information to other parties except as necessary to support
the operations of the CAM PPO.
May I change this information?
Members and providers may update their e-mail addresses and communication preference
through a link on their login welcome page. Providers may modify website URL, NPI,
and location/practice details through the Provider Profile screen. This identifying
information is transmitted between the server and the browser via a secure channel.
Changes appear in the directory immediately but are subject to review by the CAM
PPO. Providers may also update this information by calling the CAM PPO toll-free
at 866-9-CAM-PPO (866-922-6776).
